- LIKE wildcard injection: sanitize %, _ in search queries (devices, sites, gaiia)
- Jason.decode! → Jason.decode with error handling for untrusted input
- inspect() leak: replace with generic error messages, log details server-side
- SSRF protection: URL validator blocks private IPs, localhost, non-HTTP schemes
- SSRF validation added to HTTP monitoring executor and integration credentials
- GraphQL complexity limits: always applied, not just in prod

fix: handle alert_changed messages in AgentLive.Index
The StatusTitleComponent (rendered in authenticated layout) subscribes
to organization:*:alerts PubSub topic. Since LiveComponents run in the
parent process, both the component and parent LiveView receive these
messages.

Added catch-all handler to prevent FunctionClauseError when alert
changes broadcast. The component already handles the message properly.

- Fix Task.yield_many race condition causing data corruption in DevicePollerWorker
- Fix Enum.zip data corruption in SNMP Base Profile with length validation
- Fix missing Alert schema fields for check alerts (check_id, severity, etc.)
- Fix memory leaks from uncancelled LiveView timers in 4 components
- Fix PubSub subscription leak in device form credential testing

High Severity Fixes (3):
- Fix clock skew bug in needs_discovery? check with DateTime.diff clamping
- Fix nil crash in interface status display with proper nil handling
- Fix migration index names after equipment→devices table rename

Medium Severity Fixes (6):
- Fix race condition in device monitor worker (duplicate maintenance checks)
- Fix missing preload validation in devices.ex get_org_default_agent
- Fix broad rescue clause in alerts.ex with specific error handling
- Fix fire-and-forget notification tasks with try-catch error logging
- Fix LiveView state bleeding between tabs (assign_new → assign)
- Add catch-all handle_info callbacks to 3 LiveViews

Infrastructure:
- Silence health check endpoint logs (/health, /health/time)
- Add migration to fix equipment index names missed in rename

fix: sync AgentLive.Index modal state to URL for browser navigation
Update AgentLive.Index to properly handle browser back/forward buttons
by syncing the setup token modal to URL parameters.

Changes:
- Update handle_params/3 to read modal param and set modal state
- Update show_setup handler to use push_patch with ?modal=setup
- Update close_token_modal handler to use push_patch (remove modal param)
- Update handle_agent_creation_success to use push_patch after creating agent

Behavior:
- Browser back button now closes the setup modal without navigating away
- URL reflects current modal state (?modal=setup)
- Bookmarkable URLs restore exact modal state (can share agent setup URL)
- After creating agent, modal opens via URL state

Follows idiomatic Phoenix LiveView pattern using push_patch/2 and
handle_params/3 as documented in CLAUDE.md.

Fix agent page jumping by using streams for real-time updates
- Convert agent_tokens from assign to stream to prevent full page re-renders
- Update handle_info callbacks to only update the specific agent that changed
- Only update stats when agent status changes, not device counts
- Prevents IP address from temporarily disappearing during updates

refactor: consolidate device count calculations in agent_live
Remove duplicate device counting logic by consolidating three identical
implementations into a single reusable function.

**Changes:**
- Refactor agent_live/index.ex mount/3 to use calculate_device_counts/1
- Delete duplicate calculate_cloud_poller_counts/1 function
- Replace all usages with calculate_device_counts/1

**Before:**
- Lines 31-47: Inline counting for agent_tokens and cloud_pollers
- Lines 312-318: calculate_device_counts/1
- Lines 320-326: calculate_cloud_poller_counts/1 (identical logic)

**After:**
- Single calculate_device_counts/1 function used everywhere
- ~20 lines removed

**Benefits:**
- Eliminates triple duplication of counting logic
- Single source of truth for device counting
- Easier to maintain and test

feat: migrate user settings and agent management to gettext
Migrates all remaining user settings and agent management flash messages to gettext:

User Settings:
- session_manager.ex: mobile device removal, alert preferences, browser session revocation, revoke all sessions
- totp_manager.ex: device creation, verification, deletion, recovery code generation
- api_token_manager.ex: token creation, deletion, validation errors
- user_auth.ex: policy consent acceptance message

Agent Management:
- agent_live/edit.ex: agent update success
- agent_live/index.ex: agent/cloud poller creation, token regeneration, agent deletion, global default settings

All flash messages now use domain-specific helpers (t_auth, t_equipment) for proper internationalization.

Tests passing: 4165 tests, 0 failures

Now that the cloud poller checkbox issue is fixed, removing the debug output.

fix: handle checkbox 'on' value for cloud poller creation
HTML checkboxes send value='on' when checked, not 'true'. Updated
parse_agent_params to accept both 'on' and 'true' values for the
is_cloud_poller field.

Added debug logging to help diagnose future form submission issues.

Change agent revoke to delete with full cleanup
Replace the revoke functionality with delete to properly clean up agent references.
When an agent is deleted:
- All direct device assignments are removed
- Agent is removed from site defaults
- Agent is removed from organization defaults
- Devices automatically fall back to site/org agents or cloud polling

This ensures no orphaned references and provides better clarity about
what happens to device monitoring when an agent is removed.

- Add delete_agent_token/1 function in Agents context
- Update LiveView to use delete_agent event instead of revoke_agent
- Update UI button from "Revoke" to "Delete" with clearer confirmation message
- Add comprehensive tests for delete functionality and fallback behavior
- All 792 tests passing, no Credo warnings

Deduplicate agent status helper functions
Extracted 5 shared helper functions into ToweropsWeb.AgentLive.Helpers:
- agent_status/1 - Determines online/warning/offline/never status
- status_badge_class/1 - Returns Tailwind CSS classes for status badges
- status_dot_class/1 - Returns CSS classes for animated status dots
- format_last_seen/1 - Formats DateTime as human-readable time ago
- format_uptime/1 - Formats uptime seconds as days/hours/minutes

Removed duplicate code from:
- lib/towerops_web/live/agent_live/index.ex
- lib/towerops_web/live/agent_live/show.ex

Both modules now import the shared helpers, following DRY principle.
This reduces code duplication and makes status formatting consistent
across all agent-related pages.

   - Added /var/run/docker.sock mount for auto-updates in agent modal

Implement hierarchical agent assignment for SNMP polling
Add three-level agent assignment hierarchy (Equipment > Site > Organization)
allowing flexible agent deployment strategies. Agents now receive only equipment
assigned to them through direct assignment or inheritance from site/organization
defaults.

Key changes:
- Add agent_token_id to Sites table with migration
- Implement get_effective_agent_token/1 for hierarchical resolution
- Add list_agent_polling_targets/1 to return polling targets per agent
- Update API config endpoint to use hierarchical polling targets
- Add agent assignment UI to equipment, site, and organization forms
- Show agent source (direct/site/org/none) in equipment form
- Add equipment count column to agent list
- Update terminology from "poll from server" to "cloud polling"

Tests:
- Add 8 comprehensive tests for list_agent_polling_targets/1
- Add end-to-end test for hierarchical config endpoint
- All 775 tests passing
