chore(dialyzer): enable stricter flags and fix 97 resulting findings
Turned on :error_handling, :underspecs, and :unmatched_returns in
mix.exs dialyzer config. The 97 warnings this surfaced were fixed in
place rather than suppressed:

- unmatched_return (79): explicit discard with `_ = ...` for
  fire-and-forget side effects (Process.cancel_timer, :ets.new,
  send/2), and pattern-matched `:ok = ...` for control-plane
  Phoenix.PubSub subscribe/unsubscribe/broadcast calls so a future
  return-shape change fails loud.

- contract_supertype (18): tightened @spec arg and return types on
  data_builder, historical_loader, url_params, packet_utils,
  encoding_utils, aprs_symbol, weather_controller, packet_replay to
  match each function's actual success typing.

No behavioural change. mix compile clean, 1008 tests pass, dialyzer
count is now 0.

Batch push_events, RF path queries, and ETS tuning for hot paths
- Batch real-time packet push_events: collect marker data across the
  packet batch and send a single "new_packets" WebSocket message instead
  of one per packet. JS handler processes all live→historical conversions
  before adding new markers to avoid intra-batch conflicts.
- Batch RF path station queries: replace N+1 get_latest_packet_for_callsign
  calls with a single DISTINCT ON query selecting only callsign + lat/lng.
- Add read_concurrency: true to streaming_packets_subscribers and :aprsme
  ETS tables (read-heavy, rarely written).
- Remove dead GC code in PacketConsumer (threshold > 1000 unreachable with
  max batch_size of 100).

Fix code review findings: security, correctness, performance, dead code
Security:
- Add SQL identifier validation to DbOptimizer.analyze_table/vacuum_table
- Move health/readiness routes to non-rate-limited pipeline

Correctness:
- Fix monitor reference leak in StreamingPacketsPubSub (track refs, demonitor on unsubscribe)
- Persist circuit breaker half-open state transitions
- Simplify ShutdownHandler.terminate to avoid pointless Process.send_after

Performance:
- Batch marker removal into single WebSocket event (remove_markers_batch)
- Use Task.Supervisor.start_child for fire-and-forget broadcasts

Dead code removal:
- Remove copy_insert/regular_batch_insert/rows_to_csv from DbOptimizer
- Remove dead packet_pipeline_integration tests
- Remove unused telemetry event detach

- Fix Callsign.valid? docstring to match permissive behavior

- Handle login send failure in AprsIsConnection instead of crashing
- Close leaked sockets on login failure in Aprsme.Is (init + reconnect)
- Keep rescheduling keepalive timer when disconnected to prevent permanent loss
- Use supervised BroadcastTaskSupervisor instead of unsupervised Task.start
  for packet broadcasting
- Remove racy Process.alive? check in StreamingPacketsPubSub; send/2 to
  dead PIDs is a no-op, :DOWN monitors handle cleanup
- Add dateline wrapping to SpatialPubSub.point_in_bounds? matching
  StreamingPacketsPubSub behavior
- Add bad_packets table cleanup to PacketCleanupWorker

Reduce cyclomatic complexity in streaming_packets_pubsub
Extract bounds validation helpers to reduce complexity:

- streaming_packets_pubsub.ex: Extract validation helpers
  Creates all_bounds_numeric?/4, valid_latitude_range?/1,
  valid_longitude_range?/1
  Moves complex boolean conditions into readable helper functions
  Reduces complexity from 13 to within limits

Fixes 1 Credo cyclomatic complexity issue (5 remaining).

Extracts nested logic into helper functions to comply with Credo
requirements (max nesting depth 2):

- data_builder.ex: Extract historical packet data building
  (build_historical_packet_data/2)
  Reduces depth 3 -> depth 2

- streaming_packets_pubsub.ex: Extract subscriber filtering and messaging
  (send_to_matching_subscribers/5)
  Removes conditional check by sending cleanup message unconditionally
  (GenServer handles empty list gracefully)
  Reduces depth 3 -> depth 2

- historical_loader.ex: Extract RF path station loading logic
  (maybe_load_rf_path_stations/3)
  Reduces depth 3 -> depth 2

- packet_processor.ex: Extract marker popup logic
  (send_marker_with_popup_check/2)

Fixes all 4 remaining Credo nesting depth issues.

Fix CI test failures
Fixed multiple issues causing CI failures:

1. Removed unused @migration_timeout module attribute in migration
2. Fixed EncodingUtils doctests to include all fields returned by encoding_info
3. Fixed ETS table access errors in StreamingPacketsPubSub by moving cleanup
   operations back to the GenServer process (ETS tables can only be modified
   by their owner)
4. Fixed DBConnection.OwnershipError in DeviceCache by skipping database
   access in test environment

The ETS fix works by collecting dead PIDs in the async task and sending
them back to the GenServer for cleanup, avoiding cross-process ETS access.

Implement critical performance optimizations and error handling
